The forms you need to upgrade to Chartered Fellow

After you have confirmed your eligibility, you'll need to complete and return the following forms  to apply to upgrade to Chartered Fellow:

1. Chartered Fellow upgrade application form.
2. 2 x Colleague feedback forms (Support 3a to be completed by your line manager or a recent client. Support 3b to be completed by your colleague).
3. Upgrade fee payment form. The fee to upgrade to Chartered Fellow is £95.
4. Please return the above, including your current CV.  
   Please note: Additional information - we can accept a job description and/or organisation chart only. All relevant information pertaining to your application should be included in your application form and on your CV.
