Questions we should be asking employees

Good morning,

First of all apologies if I have posted this to the wrong area!!.  I am trying to pull together a standard template that can be used by our HR team in the event that someone phones us and is unable to attend work for any amount of varying reasons, all linked to the Coronavirus and self-isolating.  We are by no means medical professionals but obviously we need to gain some background information on their situation.  Has anyone done this already that could give me advice on what direct questions to ask.  I have an idea but as everything is being rushed (or seems that way at least) I dont want to miss any key points.
